What is Section 504?

Section 504 Eligibility Process Checklist
This checklist must be completed by the Intervention Specialist with each 504 Referral for Evaluation. A copy must be sent to the Intervention Coordinator along with the completed referral packet.

Referral for Evaluation
The Section 504 eligibility process may be initiated by completing this referral form and submitting it to your school's Intervention Specialist. Both school personnel and parents may make a referral. The decision to evaluate will be made by the Student Support Team (SST).

Parent Rights
The Parent Rights must be provided to the parent/guardian when the Section 504 eligibility process is initiated and annually at the review meeting.

Section 504 Evaluation
This packet documents the eligibility process for initial referrals and re-evaluations. Once students are found eligible for Section 504, they must be re-evaluated at least every three years to determine continued eligiblity.

Teacher Observation Report for Section 504 Evaluation
Each of the student's teachers must be given a copy of this report to complete and return to the Intervention Specialist prior to the Section 504 Eligibility Meeting. The SST is required to gather information from a variety of sources to use in their evaluation to determine Section 504 eligibility.

Eligibility Meeting Notice to Parents
This is the letter to the parents inviting them to participate in the meeting where the SST will conduct the evaluation (review all data collected), determine eligibility, and write a 504 plan if appropriate.

Section 504 Meeting Summary
This form may be used to document Section 504 Review Meetings. It is not necessary to document eligibility meetings on this form.

Review Meeting Notice to Parents
This is the letter inviting parents inviting parents to participate in the 504 meeting to review their child's current 504 Plan.

Section 504 Plan
A 504 Plan cannot be written for a student without documenting the eligibility process first. A Health Care Plan only needs to be completed for students who have medical conditions such as diabetes or peanut allergies.

Physician's Statement of Health Condition
This form may be completed for students who have health related impairments.

Manifestation Determination Form
The SST must conduct a Manifestation Determination meeting if a student with a disability is suspended for 10 or more consecutive school days or a series of suspensions creates a pattern of removals that result in a change of placement. School Psychologists must be invited to all Manifestation Determination meetings.

Complaint Form
This form should be provided to parents/guardians who want to file a formal complaint regarding Section 504.

Termination Form
If it is believed that a student is no longer eligible for Section 504, the SST must conduct a re-evaluation. This form must be completed to formally document the reasons for the termination of services.
